The Office of Special Education, Central Registration, and Pupil
Personnel Services is located at E.W. Miles Middle School on the north side of
the building.
To Register a New or Returning Student
- All students must be registered by a parent or legal guardian.
- To register your child, please download the necessary registration packet using the link below and fill out the forms included in the packet.
- If you cannot access a computer, contact registration by calling (631) 565-6531. Packet pick-up is available Monday through Friday from 8 a.m. to 11 a.m.
- Documents may be returned in the following ways:
- By Emailing registration@amityvilleufsd.org
- By leaving documents at the drop-off slot at the Registration Office on the North side of the Edmund Miles Middle School Building (please make a copy of your personal documents. DO NOT LEAVE ORIGINALS)
- By faxing 631-225-4614
- If you prefer to register in person, it is by appointment only, you must bring the entire registration packet completely filled with the required documents and make an appointment. Please contact the registration office via email at registration@amityvilleufsd.org or call (631) 565-6531 to schedule an appointment.
PRE-KINDERGARTEN REGISTRATION FOR 2024-2025 IS NOW OPEN
The Pre-K program at Northeast Elementary School was expanded to a full day, providing additional time for academic support, socialization, and student-teacher connections. More information is available below.
- All students must be registered by a parent or legal guardian.

Amityville UPK parents do not need to re-register for kindergarten.
In accordance with the established policy of the Amityville School District, children must be 5 years of age on or before Dec. 1, 2023, to be enrolled in Kindergarten for the upcoming year. If your child has received or is currently receiving preschool special education services, you will still need to register for kindergarten.

To Change an Address
Please follow the instructions below to submit change of address request:
- To change your address, please download the necessary packet using the link below and fill out the forms.
- Once completed please send the packet and two proofs of residency:
For Homeowners please provide the following documentation:
- Deed of Sale, Mortgage Statement or Tax Bill
- A Utility Bill
- Copy of Photo ID
For Renters or People Living with Family please provide the following documentation:
- The Landlord Affidavit - see link below (needs to be notarized)
- Proof of Residency (see below guideline)
- Utility bills such as PSEG, National Grid, Suffolk County Water Authority
- Department of Motor Vehicles or the US Post Office paperwork showing a change of address
- Documents from government agencies such as a social service agency or the Federal Office of Refugee Resettlement
- Pay Stub showing current address.
- Income tax forms that show your address
- Voter Registration documents
- Medical or Health Insurance paperwork with your name and current address

To Register New Students for Non-Public School/Home School
New enrollees must establish residency in the district by completing the following requirements.
- Complete the registration packet below.
- Attach a copy of the parent/guardian's photo ID
- If you are a HOMEOWNER, please submit a copy of a current property tax bill, mortgage statement, or house deed plus 1 current utility bill.
- If you are RENTING your home or living with family members, please complete the following Affidavit. The landlord must submit proof of ownership (property tax bill/mortgage statement), and the parent/guardian must provide 1 current utility bill.
